What do you do to minimize spammers that can join the site?

Join Date: Jun 2006 Posts: 589 Location: New Delhi, India  | I am using vBulletin on my main forum and I am using the NoSpam hack it works quite well. We do get a few manual spammers occasionally, but I usually ban their websites permanently and they don’t dare show their faces again  | ||||

But for some reason if I give them a permanent infraction or ban them they can still post?!
